01 Self-Dismissal (middle/high school students only) My child is authorized to leave school alone. I understand and accept that the school is not responsible for my child after leaving the school premises at the end of the school day. Students will not be allowed to leave during the school day without a parent or guardian. الانصراف بمفرده (لطلاب المرحلة الإعدادية والثانوية فقط) أفوض طفلي بالانصراف من المدرسة بمفرده بعد انتهاء اليوم الدراسي. أتفهم وأوافق على أن المدرسة غير مسؤولة عن طفلي بعد مغادرة مبنى المدرسة، ولن يسمح له بالمغادرة أثناء اليوم الدراسي دون مرافقة ولي الأمر. 02 Car / Private Driver My child will be dismissed to a designated car or private driver. Please provide the driver's details. سيارة / سائق خاص سيتم تسليم طفلي إلى سيارة محددة أو سائق خاص. يرجى تزويدنا ببيانات السائق. 03 Designated Person (other than a driver) My child will be dismissed to the following person. Please provide the person's details. شخص معين (غير السائق) سيتم تسليم طفلي إلى الشخص التالي. يرجى تزويدنا ببيانات الشخص.

I, the undersigned parent/guardian, authorize the school to release my child according to the option selected above. I understand that any changes to this arrangement must be submitted in writing to the school administration in advance. The school will not release my child to any person not listed on this form.
